Reds ace Hunter Greene underwent Tommy John surgery Wednesday to repair the ulnar collateral ligament in his right elbow, the second such procedure of his career after a first in 2019 in the Minor Leagues.
Manager Terry Francona confirmed Greene had a full Tommy John revision rather than the less extensive internal brace procedure, with a recovery timeline of 12-18 months expected to sideline him for the entire 2026 season and likely all of 2027.
The 27-year-old first reported soreness during an August 4 bullpen session, saw team surgeon Dr. Tim Kremchek and had an MRI the next day, then sought a second opinion from Dr. Neal ElAttrache in Los Angeles on August 6; both doctors agreed the UCL required repair.
Greene, the second overall pick in the 2017 Draft, had experienced intermittent soreness for weeks but attributed it to recovery from March 11 arthroscopic surgery that removed bone chips from the same elbow.
He has landed on the injured list in each of his five big league seasons since debuting in 2022; his best year came in 2024 with an All-Star nod and career highs of 26 starts, 150 1/3 innings and 169 strikeouts, followed by a 2.76 ERA in 2025.
Greene made just five starts this season, going 2-2 with a 6.83 ERA and 33 strikeouts over 27 2/3 innings, and his extended absence removes a planned cornerstone of Cincinnati's rotation well beyond the current postseason push.
