A fan blade in the plane's right engine broke loose, sending fragments that shattered the window and damaged the fuselage in several places, the agency's preliminary report found.
Investigators discovered bird remains inside the engine, suggesting a possible bird strike during takeoff, though they have not yet determined what caused the fan blade to fail.
Flight attendants reported hearing and feeling loud, continuous vibration and seeing smoke or fog fill the cabin before oxygen masks dropped, after which passengers noticed the man lodged in the damaged window and pulled him free.
After the rescue, the injured passenger, who suffered neck and shoulder injuries and friction burns, was moved to row 12 where an onboard doctor treated him, while other passengers used a metal box to block the opening until landing.
Maintenance records show the right engine's fan blades passed ultrasonic inspections in May 2026 and November 2025 with no damage found, though flight crews had previously reported suspected bird strikes; Greece handed the investigation to the U.S. NTSB under international aviation rules.
