Remote Care Expands Access for Sickle Cell Disease Patients, but Adult-Caregiver Disparities Persist

Caregivers of children with SCD reported markedly better home support infrastructure (80.9% versus 63.6% for adults) and were substantially more likely to give the highest rating [1]. Closing the gap will require upstream interventions: digital literacy programming, dedicated hardware access, and community health worker support integrated directly into SCD care models.
The broader clinical context deepens the urgency. The American Society of Hematology has long emphasised that SCD care is disproportionately managed through emergency departments rather than in continuous, specialist-led relationships — a pattern driven by provider shortages, implicit bias in pain management, and fragmented insurance coverage [3][4]. Patients with SCD are hospitalised more frequently than those with other chronic diseases of similar severity, and care facilities treating fewer SCD patients tend to produce worse outcomes, reflecting an experience and skills gap among providers [6]. Remote specialist consultations, when functioning well, offer a pathway to more consistent oversight that reduces emergency dependence. When they function unevenly — as the PSCRC data suggest they do for adult patients — the risk is that digital inequality replicates the same disparities that have long characterised in-person SCD care.
Looking ahead, the study's authors explicitly called for continued vigilance about differences in digital literacy and technology access as potential drivers of widening care disparities [1]. As remote consultation transitions from a pandemic-era emergency measure into a permanent feature of SCD care delivery, health systems, payers, and patient advocacy organisations must ensure that adults with SCD — who are predominantly Black or African American, frequently socioeconomically disadvantaged, and historically undertreated for pain [5] — are not left behind in a care model that currently works far better for the families of affected children than for patients managing the disease themselves [1].
